Transition from school into the after-school life through methods of person-centred-planning

The project team searched for answers to the following questions: how can we prepare the students for a future after school and how to adapt the school education for this? How to involve parents in this process? How to cooperate with other institutions in order to provide our students with better opportunities in life? How to prepare the teachers to lead the pupils to a successful life after school?

The portfolio suggests different methods of  person-centred planning, which includes collection of best practices of transition to the working life as well as recommendations about how to point out the possibilities and wishes for the students after school. We hope these materials will help teachers working with young people with disabilities for supporting them in transition to independent life and the world of work. These materials were tested in each partner country in order to improve their quality and check their relevance and applicability.
